Athletics

The following documents are required ANNUALLY to participate in athletics at Harvest Community School:

1. EL2 – Sports Physical
2. EL3 – Consent and Release
3. NFHSlearn.com Mandatory Training Certificates (Must submit 3 certificates after completing the following online courses: Concussion for Students, Heat Illness Prevention, Sudden Cardiac Arrest)
4. Warrior Athletic Information Form (Includes checklist of necessary items/forms)
5. Athletic Handbook (Don’t need to print, just initial page 2 of the Warrior Athletic Information Form)
6. For FOOTBALL and JV & VARSITY VOLLEYBALL athletes: SSAA Form CRF-1
7. GA4 – Required for First Year Athletes only
8. Non-Traditional Students require the following in addition to the above documents:
   1. *Attend an Informational Meeting
   2. *Meet with our AD, Jackson Wilcox and/or our founder, Patty Wilcox.
   3. *Non Traditional Student Agreement Form (Includes checklist of necessary items/forms)
   4. *Homeschool Students Registered with the County:  EL7/EL7V
   5. *Homeschool Students Receiving PEP Scholarship – EL15
   6. EL9 – Due the first week of January
   7. *Copy of Transcripts
   8. *Copy of Birth Certificate – (if guardian is not on Birth Certificate, please provide document showing legal guardianship)
   9. *Non-Traditional families must set up a FACTS Management account within the first week of practice. HCS uses FACTS to bill athletic fees.  Once you set up an account, Create the account (username and password) and then go to “Set up a Payment Plan”.  Enter Student info and then click on save and exit.  You do not have to enter any form of payment. 
   10. *Other forms may be needed based on individual situation.
