The following documents are required ANNUALLY to participate in athletics at Harvest Community School:

1. EL1 – Florida High School Athletic Association
2. EL1/2S – Florida High School Athletic Association (if medically necessary) Florida High School Athletic Association
3. CRF-1 – CRF-1_Form_2024-25.docx.pdf
4. EL2 – Florida High School Athletic Association
5. EL3 – Consent and Release
6. NFHSlearn.com Mandatory Training Certificates (Must submit 3 certificates after completing the following online courses: Concussion for Students, Heat Illness Prevention, Sudden Cardiac Arrest)
7. Warrior Athletic Information Form – please contact us for this form
8. Athletic Handbook – please contact us for this information
9. For Football, Volleyball, Basketball, Soccer and Softball athletes: SSAA Form CRF-1
10. GA4 – Required for First Year Athletes only
11. Non-Traditional Students require the following in addition to the above documents:
    1. *Attend an Informational Meeting
    2. *Meet with our AD, David Meddings and/or our Executive Director, Jackson Wilcox.
    3. *Homeschool Students Registered with the County:  EL7/EL7V
    4. *Homeschool Students Receiving PEP Scholarship – EL15
    5. EL9 – Due the first week of January
    6. *Copy of Transcripts
    7. *Copy of Birth Certificate – (if guardian is not on Birth Certificate, please provide document showing legal guardianship)
    8. *Other forms may be needed based on individual situation.
